How to Create a Survey (Written Instructions)
To create a survey:
- Go to your CourseNav bar and click on Surveys. You can also go to the Course Admin tab on your navigation bar to access Surveys.
- From the Surveys tool, click New Survey.
- Enter a name for the survey.
- In the Anonymous section, select “make results anonymous” if you want to hide user data and survey results. If you create an anonymous survey, you cannot add Release Conditions. Release Conditions can only be added to a survey where the respondent is known.
- To add questions to your survey click Add/Edit Questions.
- Click New.
- Select the question type.
- Enter the questions Introductory Text to provide instruction to students.
- Select the scale you would like respondents to use when rating items.
- Enter the items respondents will be rating into separate value fields.
- Click Preview to view how your survey will display to respondents.
- Click Done to continue editing the question.
- When ready, click Save.
- Click Done Editing Questions to return to the Survey page.
- Click the Restrictions tab to modify the surveys availability.
- To make your survey available to users click the status drop down list and select Active. You may also wish to specify a date range for the survey in the In the Dates section.
- Click the Attempts Allowed drop down list to set the number of attempts respondents can complete
- When your survey is ready, click Save and Close.
